Employers offering a wellness program are required to provide employees with a notice informing them of what information connected to the wellness plan will be collected by their employer, how that information will be used, who will receive that data, and how the employer plans to keep that personal information confidential. This requirement has to be fulfilled to remain in compliance with the Americans with Disabilities Act (ADA).
